Gwynedd Hospice at Home was registered as a charity in 1990 following a successful appeal to the general public for
£500,000 to establish and provide a 'Home Nursing Service' for patients in the old county of Gwynedd. The service became operational in 1992 and following its development, the Hospice Nursing Service is now working within the Department of Palliative Care of the North West Wales NHS Trust alongside their Medical
and Nursing Colleagues, Occupational Therapists and Physiotherapists,
all of whom work closely with the Primary Health Care Teams.
The Charity also
provides support to help patients live as actively as possible and to help the
family cope both during the patients illness and in their bereavement. The Day Hospice is an integral part of this support and this centre is located on the Ysbyty
Gwynedd site in Bangor (Hafan Menai).
